Sprout Health Arizona provides evidence-based, clinically-proven and remarkably effective treatment programs to those who are suffering from drug addiction, mental health illness and co-occurring disorders. We understand the stress this situation causes potential clients and have created an effortless pre-admission and intake process. We accept referrals from multiple healthcare organizations and, with our numerous resources that are designed to aid both clients and referrers, we help facilitate a seamless transition to our facility. Our resources will be available during the admission, treatment and discharge phases in order to keep open communications between our facility, patients and the referring health care provider.

We pride ourselves on our fast and straightforward communication in order to properly receive the necessary documentation and medical information regarding prospective clients. Our intake and referral coordinators are equipped to formulate unique and highly individualized treatment plans for each potential client. The goal at Sprout Health Arizona is to provide unmatched customer service to consolidate what would normally be a challenge to the currently accepted health care referral process.

The Admission Process

Sprout Health Arizona understands that the majority of common patient admissions systems are slowed due to regulatory processes and we strive to relieve those challenges. We provide all referring individuals, professionals and organizations with accurate information and guidance pertaining to our treatment centers and programs.

Once a client chooses to receive treatment from Sprout Health Arizona, we will contact the referring health care professional to gather as much necessary information pertaining to the client in order to develop a custom treatment plan catered specifically to an individual’s particular needs. We take into consideration health and addiction history as well as any underlying conditions that may have been diagnosed during our assessment stage.

We pay strict attention to every detail in order to streamline the admission and intake process. We maintain legal and ethical integrity as well as honor patient confidentiality and the Health Insurance Portability and Accountability Act.

Disclosure and Patient Consent

Due to HIPAA and other laws regarding consent, our intake coordinators request clients to authorize the disclosure of their personal health information. Contributing medical professionals may want updated information on the client’s progress; however, some information cannot be disclosed; therefore, we will request written consent from the client. We respect referrals from the aforementioned medical professionals; however, we also respect our client’s right to privacy.

If clients give consent for their information to be disclosed, our referral coordinator will notify the referrer of the patient’s treatment and discharge strategy. All authorized professionals will be updated within 72 hours after admission to the facility in order to discuss a strategy for future updates.

Planning for Discharge

Sprout Health Arizona’s goal is to provide successful treatment and long-term recovery to each patient. We ensure our patients continued care after they are discharged from our facility and notify authorized professionals of their discharge plans. Our referral coordinator will also communicate the arrangements to the referring health care provider. The goal of discharge planning is to prepare our clients for their release from our facility as well as notify health professionals of their impending release. We want to prepare everyone for what is to come. Our hope is that every client leaves with the tools necessary for sobriety and mental wellness. Continued care can significantly increase the chances of a successful recovery; therefore, the referrer should prioritize the continued support the client may need in order to fortify their treatment.

We strongly believe that the solution to substance abuse, addiction and mental illness requires cooperation from the patient, referring health care and society in general. By streamlining and eradicating the abundance of obstacles that arise during the admission and referral process, we are better able to serve the clients who are seeking mental health or substance abuse treatment. Sprout Health Arizona greatly appreciates the partnership and assistance of referring health care professionals; without them, we could not provide our level of treatment.
