The Pre-Admission Process
The decision to live a life of sobriety and get treatment for you addiction can be difficult; however, once made, you do not have to do it alone. Our friendly staff will be there to help you through not only the admissions process, but the entire treatment and years to follow as well.
After contacting Sprout Health Arizona, our trained pre-admission representatives will work with you either over the phone or during a visit to our facility, to gather information in order to facilitate preliminary treatment plans relevant to your condition. This may include a history of your addiction, such as symptoms, behavioral issues and frequency of use, which will allow our team of experts to set your admission date.
Setting your admission date marks the beginning of your journey to sobriety and regaining control over your life. We understand that while this is an exciting time, it is also stressful for you and your family. We want to ensure you that we have your best interests at heart and want to make this process as easy as possible. You and your family will receive a Family Welcome Packet, which is an informational resource that includes everything necessary to know all about Sprout Health Arizona, such as contacts and programs we offer.
Welcome to Sprout Health Arizona
You will arrive at our facility on the admission date set for you during the pre-admission stage. One of our friendly and supportive intake members will excitedly greet and provide you with an overview of the treatment process, existing rules and regulations and the policies and procedures of the treatment facility. Our rules are designed for your safety and well-being but are in no way meant to hinder your basic freedoms.
During the admission stage, you will also be assessed in order to determine primary and underlying issues, which will help us prepare the most effective and individualized treatment plan for your personal needs. For first-time clients, you will also undergo a comprehensive psychiatric and medical evaluation to determine if existing co-occurring conditions are present. Sprout Health Arizona aims to provide specific therapeutic approaches that are appropriate for your specific needs and goals.
Useful Documents You Should Bring
During intake, you will need to provide the following documentation, which will form part of your Financial Aid Packet:
- Valid identification card issued by the government, which includes a clearly identifiable photo (e.g. driver’s license or passport)
- Completed credit card authorization form
- Photocopy of insurance card along with the original for verification
You may use an insurance program of your choice; however, the Affordable Care Act now covers mental and behavioral health treatments, such as addiction treatment. If you decided to use another insurance, you can discuss it with our intake team.
Power-of-Attorney, Assignment of Benefits and Insurance Financial Agreements are also needed; however, they must be signed in the presence of a certified Notary Public. Additionally, consent forms will be discussed and signed by you and the intake team. These forms are especially important to ensure your privacy and confidentiality. Your information will only be released to authorized members of your family or those directly involved in your treatment.
If your treatment is tied to a felony, you will need to present court documents as well as undergo a mental health evaluation and drug test.
